The main aim of clinical pharmacokinetics is to optimize pharmacotherapy. This requires an in-depth knowledge of the pharmacokinetic parameters and different population-based models.

Its application requires a global vision that takes into account the state and clinical characteristics of the patient and the pharmacotherapeutic objectives.

The most commonly monitored drugs are antimicrobial agents, digoxin, antiepileptics and immunosuppressants.

Pharmacokinetic monitoring is particularly useful in specific populations or clinical situations, such as abnormal liver or kidney function, extreme ages or hypoalbuminaemia.

Clinical pharmacokinetics units at hospitals are essential to ensure personalised medicine.
